Skilled Nursing Facilities

Patients may come to sub-acute rehab (SAR)  following a surgery or hospitalization. These patients get intensive physical therapy and occupational therapy, with a goal of regaining strength and function that may have been lost during hospitalization. Nurses are present around the clock to provide care. Other services provided in this setting include administration of IV fluids or IV antibiotics, wound care, lab draws, x-rays, and bedside ultrasounds. Some facilities also provide in-house dialysis. Once patients have completed their rehab services, most are discharged home. Some transition to long-term care if they cannot be cared for safely at home. 

For patients requiring long-term nursing care, the skilled nursing facility becomes their permanent residence. These patients have access to in-house lab work, medication administration, x-rays and bedside ultrasounds. We provide their primary care and health maintenance visits, and help get them scheduled and transported to outpatient services or specialists.
