Q: I'm having a surgery and will need rehab afterwards. How can I be sure that I come to a facility where Endeavour Healthcare can take care of me?
A: There are many factors that impact which rehab facility you go to after a hospitalization or surgery. Working with the hospital social worker is the best way to make your wishes known. The social worker can reach out to our group in order to coordinate a plan.
Q: My family member was in sub-acute rehab but needs to transfer to long-term care. Will Endeavour Healthcare still care for her?
A: Absolutely. Continuity of care helps to improve patient outcomes and maximize health. We care for patients both in the short-term rehab setting as well as those requiring long-term care and placement.
Q: My family member is currently in a skilled nursing facility but assigned to another doctor. Can we switch? What if they are in a facility where you don't currently see patients?
A: Yes -- patients/family decision-makers can always request a change of physician. If you or your family member are in a facility where we do not currently see patients -- ask to speak to the facility administrator about having us join.
Q: After I get out of rehab I would like to continue to have Endeavour Healthcare be my primary care provider. Do you have an office?
A: Not yet -- but we hope to expand to an outpatient office space within the year. Check back on our website for updates!
